What IS Care Management?
A Care Manager Defined
- Every child referred to Bergen's Promise will work with a Care Manager who helps manage, coordinate and access resources and supports needed in the lives of the child and family.
- The Care Manager will meet personally with the child and family to help identify members of their Child and Family Team and other important people in their lives.
- They will help to coordinate the initial meetings of the Child and Family Team.
- The Care Manager will work with family members to learn their concerns, identify their needs and acknowledge their strengths.
- In addition, the Care Manager will contact the Family Support Organization which will provide a partner to help support and advocate for the parent or caregiver.
The following quotes are from the very special people, the Care Managers, who truly "care" and "manage" the lives of young people who are part of Bergen's Promise:
"When the children and their families succeed, they are the
success, not us."
"We function as an
advocate for the child, and liaison to the community. The focus is on the building of strengths within a child, not what they have done wrong."
"We're the connector, coordinator, nurturer, mentor, and encourager."
"We're a bridge from the parent to the child to the community to resources and supports."
"Being a Care Manger is like being a megaphone. We take the voices of the families who are lost, struggling or not being heard and amplify them so that all of Bergen County can hear in the hope that somewhere, somehow, someone will answer them."
