What IS Care Management?

A Care Manager Defined

  • Every child referred to Bergen's Promise will work with a Care Manager who helps manage, coordinate and access resources and supports needed in the lives of the child and family.
  • The Care Manager will meet personally with the child and family to help identify members of their Child and Family Team and other important people in their lives.
  • They will help to coordinate the initial meetings of the Child and Family Team.
  • The Care Manager will work with family members to learn their concerns, identify their needs and acknowledge their strengths.
  • In addition, the Care Manager will contact the Family Support Organization which will provide a partner to help support and advocate for the parent or caregiver.
