Care Management Organization (CMO)
Another essential piece of the puzzle
Bergen's Promise doesn't stand alone. The agency is just one important component of a larger system of care within New Jersey's Division of Child Behavioral Health Services. The CMO is designed to provide Bergen County youth with quality mental health care in their own communities. County based Care Managent Organizations throughout New Jersey coordinate a full range of treatment and support services to children with the most complex needs.
Those youth with the most urgent and complex needs are placed in the Care Management Organization under the supervision of a Care Manager. Each Care Manager serves a maximum of 10 children.
The Care Manager is responsible for facilitating and coordinating the Child and Family Team meeting and developing an Individualized Service Plan (ISP) with access to an array of services deemed most beneficial to address the child's needs.
There are 18 Care Managers at Bergen's Promise providing services for 180 children. Three supervisors provide continuous direction and clinical expertise and the Clinical Director oversees all plans to ensure that they are thorough and in compliance with mandated guidelines.
The model that underscores the philosophy of the Care Management Organization, Wraparound, essentially "wraps" services and supports around the child and family in a responsive and individualized manner.
