Provider Information

Bergen's PROMISE is designed to access formal and informal supports and services for these children and their families. The services are community based, individually tailored and include in-home clinical services. They are flexible and outcome based with services, management and decision making responsibilities resting at the community level.

Bergen's PROMISE is the Care Management Organization designated to innovate and implement approaches to a wide array of effective community and grass roots services and service delivery. Our philosophy is that service plan development for children with serious emotional and behavioral challenges is an interactive process and a collaborative effort between the family, child and community providers. Service planning is based upon the "wraparound" model, a process which recognizes and builds on the strength of the family and community to devise individual strategies to help children lead lives as normal as possible.

We believe that the most effective supports and services are delivered in the community.

Community based organizations help us help families increase their choice and independence. We develop affiliation agreements and encourage partnerships within our network of providers. Funds to pay for non-traditional services are also available.
