Part Time Wellness Educator - Temp Position

Location:
Compensation: $33 per hour | Part-Time

Overview:

This is a part time, grant funded, temporary assignment totaling 22 hours per week for 12 months.

Responsibilities:

As a part time Wellness Educator, you will have the opportunity to:

  • Participate in the development of self-management health goals for youth up to age 21 and their family to be included in the service plan
  • Facilitate regular face to face meetings with youth/family to monitor progress and provide support for the self-management goals included in the service plan.
  • Engage assigned child and families in health promotion planning and activities, including the provision of motivational interventions to increase treatment and support lifestyle changes.
  • Provide health education specific to the 8 Dimensions of Wellness Model of assigned youth.
  • Facilitate the engagement of youth and families in community supports which may include preparing youth and family for pending appointments and accompanying youth and families on appointments to reduce youth/family apprehension.
  • Participate in transitional care and follow-up planning on assigned youth.

Qualifications & Skills:

To be considered for this role, you must possess:

  • A Bachelor’s Degree in public health, health education, health promotion OR related field; and certification appropriate to field of study as applicable 
  • A high level of leadership and professionalism
  • The ability to work with energy and enthusiasm on assigned teams
  • Computer literacy and proficiency in writing skills
  • A valid driver’s license and vehicle insurance
