Program Supervisor - Care Management

Compensation: $63,000 - $68,000 | Full-Time

Overview:

Bergen's Promise is seeking an experienced Program Supervisor. This position is responsible for the supervision and oversight for a team of six Care Managers who provide crisis management, referrals and linkages to services for the youth and families we serve.

Opportunities such as participating in one of our internal committees that support the Mission of Bergen's Promise, participating as a Field Supervisor in our Internship program or by developing and presenting internal training to other members of our team.

Responsibilities:

  • Responsible for the recruitment and retention of high-performing Care Managers.
  • Responsible for the professional development, the administrative and clinical case oversight of a team of six Care Managers, who provide access to clinically-appropriate and individualized interventions designed to meet the needs of their assigned youth and families.
  • Ensures the execution of the organization's mission - Healthy Families, Safe at Home -by promoting a culture of exceptional service, strong and coordinated community partnerships and strength through accountability.
  • Accountable for the Care Manager deliverables, which include assessment, development of an individualized plan of care, linkage and referral to services, and monitoring for quality service provision.
  • Tasks are executed primarily in an office environment, though some supervisory responsibilities require home- and community-based outreach.

Qualifications & Skills:

  • Master's Degree in Social Work, Counseling or related field.
  • Current and Valid New Jersey license is a state requirement: LSW, LCSW, LPC, LMFT required - candidates who do not possess a New Jersey license will not be considered.
  • LCADC license a plus.
  • Experience with individuals (preferably children) with substance use disorders, and/or intellectual and developmental disabilities a plus.
  • Minimum one-year experience providing supervision to a team of direct reports.
  • Bilingual a plus.
  • Prior experience with at-risk children a plus.
  • Work additional hours as necessary.
  • Basic computer proficiency.
  • Exemplary time management and organization skills.
  • Excellent verbal and written communication skills.
  • Culturally Competent.
  • Valid Driver's license required.
