  • Care Manager

    Rochelle Park, NJ
    Full-Time | $37,000 / year

    Bergen's Promise is the Care Management Organization (CMO) for Bergen County. We are a non-profit social service agency serving youth ages 5 - 21 and their families. The youth served are challenged by serious behavioral, emotional, mental health, substance abuse issues and/or developmental disabilities.

    We are currently seeking Care Managers to join our winning team in a fast-paced work environment. The Case Manager position requires care coordination and crisis work, in community, with children and families in Bergen County NJ.

    Must be mission driven, creative and able to work under supervision and as part of a team.

    Must be able to work a flexible schedule to accommodate the needs of the youth and families served.

  • Pediatric Community Health Nurse - Per Diem

    Bergen's Promise is looking for a Per Diem nurse to perform nursing assessments for youth who have been referred for much needed Behavioral Health services.  By assisting us with these nursing assessments you will be providing these families with the opportunity to become healthier, to live more productive lives, to be the best version of themselves.

    These assessments include obtaining applicable metabolic levels and information for assigned youth from primary care providers and school nurses if applicable.  You will also be responsible for ensuring that documentation is posted within the electronic record and filed in the hard chart within the required time frames and compliant with applicable regulatory standards.


    Approximately ten hours a week - late afternoon and early evening hours only - with the exception of initial training.

  • Social Work Supervisor - Bilingual

    Full-Time | $63,000 - $68,000 / year

    Bergen's Promise is seeking an experienced Spanish-speaking Social Work Supervisor. This position is responsible for the supervision and oversight for a team of six Care Managers who provide crisis management, referrals and linkages to services for the youth and families we serve.

    Becoming a member of the team at Bergen's Promise will provide you with opportunities to develop yourself further as a supervisor and manager as well as a professional in the social service arena. Opportunities such as participating in one of our internal committees that support the Mission of Bergen's Promise, participating as a Field Supervisor in our Internship program or by developing and presenting internal training to other members of our team.

    What we expect:

    • Responsible for the recruitment and retention of high-performing Care Managers.
    • Responsible for the professional development, the administrative and clinical case oversight of a team of six Care Managers, who provide access to clinically-appropriate and individualized interventions designed to meet the needs of their assigned youth and families.
    • Ensures the execution of the organization's mission - Healthy Families, Safe at Home -by promoting a culture of exceptional service, strong and coordinated community partnerships and strength through accountability.
    • Accountable for the Care Manager deliverables, which include assessment, development of an individualized plan of care, linkage and referral to services, and monitoring for quality service provision.

    Tasks are executed primarily in an office environment, though some supervisory responsibilities require home- and community-based outreach.
